Create surveys in Evo Survey

Build, publish, and analyse surveys in Access Evo.

When you use Evo Survey with the Survey creator role, you can build new surveys, send them to your users, and analyse their replies.


Create new surveys

To create a new survey, follow the steps below:

  1. In Access Evo, click Access Button

  2. Click Productivity Tools, then select Evo Survey.

  3. Click Survey creator, then click New Survey.

  4. Enter a title, and if required, a description and cover image.

    🤓 Tip: Use a title that’s clear and easy to search for:
    ​✔️ Good: Evo features training feedback survey 2026.
    ​❌ Bad: Sarah’s survey.

  5. From the left-hand menu, select your question type.

    • To make the questions mandatory, select Required.

    • To delete a question, click the bin icon on the right.

    • To copy a question, click the duplicate icon.

    • Rearrange the questions using the up and down arrows.

  6. Enter your questions, and answer options if required.

  7. Click Next: Logic Flow, then review how your questions lead on from each other.

    • To amend your logic flow, click Back, then make the changes.

  8. Click Next: Settings, then fill in the options:

    • Enter response deadline.

    • Who can respond to your survey.

    • How the answers appear.

    • Who the survey shows it’s from.

    • If users can edit their answers after submitting.

    • When it’s available.

  9. Click Publish Survey.

  10. Select recipients then enter an optional message to them.

    • You can send the survey to yourself too.

  11. Click Send Feed notification, then click Finish.

🤓 Tip: Your recipients can find the survey notification in Feed, or they can open it in the Evo Survey productivity tool.

Format the cover image

Once you’ve selected a cover image, you can edit using the options below:

  • Move: Click and drag the image to place it where you want it.

  • Remove: To remove the image, click Remove.

  • Change: To pick a different image, click Change, then select a different image.

  • Zoom: Use the Zoom slider to zoom in or out of the image.

  • Crop: To cut out a section of the image, click Crop, select the area to keep, then click Apply.

  • Height: To change how thick the banner cover image displays, select either Small, Medium, or Large.

🤓 Tip: When the survey is live, the title, description, and deadline appear over the image on the left-hand side. So try not to use an image where text or the focus of the image is on the left.

Question types

There are many different question types that you can use in your surveys. For more information on what these are and how they work, see the below:

🤓 Tip: All questions are 500 characters, including spaces. To add more context or information, click the three-dot menu icon, then click Subtitle. You can now add up to 1000 characters. To remove it, click the three-dot menu again, then click then click Remove subtitle.

  • Short Text: 1000 character answer, including spaces.

  • Long Text: 2000 character answer, including spaces.

  • Multiple Choice: The user can select more than one option.

    • Provide up to 10 answer options.

    • To add more options, click Add option under the list.

    • To remove options, click the X on the far right.

    • To shuffle the options, click the three-dot menu icon, then click Shuffle options.

  • Checkboxes: The user can only select one option.

    • Provide up to 10 answer options.

    • To add more options, click Add option under the list.

    • To remove options, click the X on the far right.

    • To shuffle the options, click the three-dot menu icon, then click Shuffle options.

    • To add branches, click the three-dot menu icon, then click Add branching.

      • For each answer option, click the Go to list and select what you want to happen if they choose that option. You can send them to the next question or end the form.

  • Rating (1-5): Let your users select a satisfaction score from 1-5.

    • 1 is very dissatisfied, and 5 is very satisfied.

    • To enable a follow-up question, select the toggle on the far right, then enter your question.

    • To add branches, click the 3 dots, then click Add branching. For each answer option, click the Go to list and select what you want to happen if they choose that option. You can send them to the next question or end the form.

  • NPS: This is a New Promoter Score. Your users can select from 0-10 to show how likely they are to promote something.

    • 0 is Not likely at all, and 10 is Extremely likely.

    • To enable a follow-up question, select the toggle on the far right, then enter your question.

    • To add branches, click the three-dot menu icon, then click Add branching.

      • For each answer option, click the Go to list and select what you want to happen if they choose that option. You can send them to the next question or end the form.

  • Date: Let your users select a date.


Navigate the My Surveys dashboard

All surveys you’ve created show on the My Surveys dashboard. Select any of the tiles to see the surveys that fall into that category:

  • Total: A list of all your surveys, regardless of status.

  • Active: Live surveys that you’ve sent.

  • Draft: Surveys you’ve started to create but not sent.

  • Closed: Historic surveys that are past their expiry.

Each survey on the list shows the title, status, how many people have complete it, when it was published, and the expiry date.

🤓 Tip: To see a preview of the survey, click the three-dot menu on the right, then click Preview.


Results

To see the survey answers and response rate, follow the steps below:

  • From the left-hand menu, click Results.

  • Your surveys are listed; they show the deadline and response rate.

  • On the required survey, click View Responses.

  • To download the results, click Export CSV.

Review the data

On the Results dashboard, you’ll see:

  • How many responses you’ve had.

  • The completion rate.

  • How many days are left before the deadline.

  • When the responses came in.

  • All answers to the questions.
